失效模式與效應分析(Failure Mode and Effects Analysis, FMEA)是一種防止失效的結構系統分析方法,著重於錯誤還未發生之前就加以預防及修正。本研究以晶圓切割機(Wafer Dicing Machine)為例,應用FMEA法,針對晶圓切割機的切割機構進行探討,找出可能會發生的失效模式與可能發生之原因,評估嚴重度、發生度及難檢度,依照風險優先指數去決定解決問題的優先順序,建立預防機制來延長晶圓切割機的使用年限、縮短維修時間及停機時間。藉由失效模式與效應分析,從晶圓切割機16種可能失效模式中,找出5項風險優先指數最高的失效模式進行小組討論,提出改善措施,其中風險優

先指數最高達144,經過改善後降低至30,其餘項目的風險優先指數皆有下降。
