背景:若未經移植,心臟衰竭是種不可治癒之慢性疾病。疾病發展至末期變化大,症狀困擾影響生活品質、功能受限與存在安適;代償失衡之下死亡較難預測。國內尚缺乏末期心衰竭病人症狀困擾、生活功能和存在困擾軌跡變化的資料,末期緩和醫療照護備受挑戰。目的:本研究旨在探討嚴重心臟衰竭病人症狀困擾、日常生活活動、存在困擾半年內的變化,並分析此變化對一年內死亡的預測。方法:採前瞻性追蹤的研究設計,於台灣兩家南部醫院門診以方便性取樣,招募左心室射出率(LVEF)≦30%、紐約心臟學會功能分級(NYHA)IV或美國心臟學會心臟衰竭D階段、治療後仍有症狀且一年內曾住院或急診的原發或結構性心臟病患,排除接受心臟移植後共7

1位樣本。資料本共追蹤2、4、6月的生理症狀、日常活動功能與失志,同時輔以病歷的疾病、治療與12個月內死亡或存活的資料收集。研究結果:本樣本平均年齡為52.0歲,多數為男性76.1%、高中職 40.85%、NYHA Fc III 70.4%、LVEF平均23.6%。雖病人的生理、心理和心衰竭症狀困擾偏中低嚴重度、日常生活活動受限偏低、失志為中等程度,但內部個別差異大。四次測量中,約半數抱怨口乾、咳嗽、睏倦、疲憊與擔心,而且其發生頻率隨時間增加而至65%以上;高失志者(≧30.27分)占43.7%至56.1%。廣義估計方程式分析顯示,6個月後的症狀困擾與失志顯著的增加;但專科大學、工作組的失志程

度顯著較低。在其它共病的調整之下,12月內的死亡風險隨心衰竭症狀困擾嚴重度而增加,且第4、6個月的失志增加、女性較男性的日常生活活動功能降低亦會增加死亡的風險。討論:症狀困擾與存在困擾是嚴重性心臟衰竭病患常見的問題,而且其嚴重度在6個月內隨疾病時間增加而增加,甚至增加一年內的死亡風險。故臨床實務上應定期與持續評估嚴重心衰竭病人生理心理與活動的狀態,適時提供緩和醫療照護措施。
